J.C. Lewis Primary Health Care Center, Inc., excels in compassionate care. We encourage healing through healthy living.
Our Health Services programs are efficient, effective, community-based, patient-directed and conveniently located throughout downtown Savannah.
Our programs give hope to populations with limited access to medical, behavioral, dental, medical respite care and housing. Individuals and families included are low-income, uninsured, homeless and/or living with HIV/AIDS.
History
- 1999-J.C. Lewis Outpatient Clinic was established to address the healthcare needs of chronically homeless persons. JCL was designated as a Health Care for the Homeless site.
- 2004-J.C. Lewis Outpatient Clinic became a Federally Qualified Health Care Center providing to address the healthcare needs of Health Care for the Homeless men, women, and children.
- 2007-Peter Brasseler Dental Center was established to address the oral needs of homeless, uninsured, and underinsured persons.
- 2009-J.C. Lewis Primary Health Care Center Outpatient clinic expanded into a Community Health Center to address the healthcare needs of ALL men, women, and children. We accept Georgia Medicaid, Well Care, Amerigroup, Peach Care for Kids, MasterCard, Visa, Debit Cards and Cash.

Eligibility
Everyone is welcome to visit our healthcare programs. However, program patients and/or residents are required to provide documentation to receive discounted fees or participate in discount programs for services rendered. The following items are required for registration:
- Verification of current address
- Verification of insurance if applicable: Medicaid/Medicare card, Insurance Card
- Verification of income: pay check stub, benefits statement, DOL W-G 15 letter, Case Manager signed income form
