Wellness Annual Exam

Let J.C. Lewis Pediatrics help keep your child healthy with regular exams.



Sports Physicals

Get your child’s (or teen’s) sports exam at J.C. Lewis Pediatric Center



EED Form3300 EOA

We will check your child for vision, hearing, teeth and nutrition. Grade school and Head Start.



Immunizations

Keep your child’s Immunizations up to date. We will vaccinate your child and answer all of your questions.



Therapy

Schedule our Licensed Clinical Social Worker to meet with your child. For children ages 10 years old and up.



WIC

We can get you the prescription you need to start the Special Supplemental Nutrition Program for Women, Infants, and Children.



Dental Clearances

Schedule your appointment for a dental clearance.



Reach Out and Read

Reach Out and Read promotes reading aloud and provides books at regular check-ups.
